Vial of Life Program
Vial of Life is a non-profit organization that has developed a way to
provide documentation to emergency responders when you are not able to speak
for yourself. It consists of filling in a form with critical medical
information and placing it in a baggie on your refrigerator door. There is a
label associated with the baggie identifying it as containing "Vial of
Life" information. There is a matching label that you can place on your
front door so emergency responders know to look on the refrigerator door for
the baggie with the needed information in it.

If you use their website, you have the option of filling out the form and
printing it online or saving it to their server for future editing. You can
also print the necessary labels from their website.
If you are uncomfortable saving your medical data online, we have created a
Microsoft Word document that contains all the needed fields that you can save
to your local PC and either fill it in "live" and print it or just
print a blank copy and fill it in by hand. If you fill it in "live",
you can then save it to your local drive and open it for editing as your
medical history changes. That way, you don't have to fill in all the data every
time and your data is only available on your own PC.
Although you will be able to open the form and edit it with WordPerfect, you
loose the advantage of using the tabs and the form fields. If you have
Microsoft Word on your PC, the form incorporates features that will make it
easier to enter the data. The form is available in the navigation menu to the
left under the heading "Our WORD form".
For your convenience, the labels are available from this site. USe the
navigation menu and select "Print Labels" to access the label page.
If you have a color printer and can print on photo paper, you will get a bright
and very visible label.
